Login problems, continuously getting the same message

We had the same issue with Microsoft MFA enabled on a Global Admin account and were using LastPass TOTP as the 2FA token. However, if you drill into Conditional Access > Authentication strengths and look at the type of auth methods being allowed, that was our issue. We didn't 'Password + Software OATH token' specified and this put us in an infinite loop. The default 'Multifactor authentication' option as of now, has all auth methods specified, it may be worth making sure your policy is set to use that to see if the loop issue persists.
We fixed the issue by changing the Authentication Method configuration for Microsoft Authenticator from "Passwordless" to "Any".
Entra home -> Protection -> Authentication Methods -> Microsoft Authenticator -> Authentication Mode = "Any".
