SharePoint List Titles not showing

I had the same thing happen after copying a list on the same site. Here's I fixed mine:

1. Enable Allow management of content types. 
2. Open the Item content type for editing.
3. If you don't see Title in the column listing, click Add from existing to navigate to the Add Columns screen.
   1. In my case, Title was the only column showing in the list. Select it, Add, then OK.
   2. While still in the List Content Type screen, edit the column order to place Title at the top where it belongs.
   3. You should now be able to disable management of content types and test the form.
4. If Title is already showing in the column listing for the content type, you could try clicking it to open, then ensure it's not hidden.
